Discern Realities is a Dungeon World move used to investigate a situation and closely study a person or environment. When a character examines their surroundings carefully, they roll 2d6 plus their Wisdom modifier. The move allows characters to learn more about a scenario or situation through direct observation, rather than about abstract topics or knowledge (which would be covered by Spout Lore).
